2009/10 Coach and Program Evaluation

The above link will take you to the 2009/10 Evaluation on Survey Monkey.  This is the first year for collecting responses this way.  It is designed so that it is filled out one per player.  The Program Evaluation at the end needs to be filled out only once.
Data will be collected until April 24. 2010.
